Services: provide the necessary medical support for patient who is terminally ill and also support for the family.

Process: A call to request services can be made by a family member, doctor, friend, or person who is terminally ill. The

agency will need to obtain a referral from the doctor in order for services to be provided.

Information is first taken over the phone and the assessment is completed by a visit to the individual who is sick

Hospice provides the patient’s required medication and medical equipment depending on the diagnosis

Bereavement Support Group: family members can attend during patient’s illness and upon patient being deceased.

Time of group varies depends on availability of family

Chaplin also available

Agency will follow-up with family up to 13 months after the loss
